Illuminate Hospice is a Medicare/Medicaid-certified and CHAP-accredited provider offering comprehensive hospice care in Illinois. Medicare and Medicaid cover hospice care entirely without copays, and we also accept most private insurance plans that provide hospice benefits.

Physicians
Our expert hospice care team will frequently assess your needs and pain level. Your nurses and aides will work collaboratively with your physician to manage your pain and relieve symptoms, allowing you to live each day as fully and comfortably as possible.
Registered Nurses
Experienced, skilled Registered Nurses provide hands-on care and continually assess your needs to manage your pain and symptoms. The nurses also provide support and education to you and your family.
Spiritual Counselor
Our non-denominational Spiritual Counselors and Chaplains support you and your loved ones during the hospice process, helping guide you through the tough end-of-life conversations. We can also coordinate with your clergy or any other spiritual support person you want to include.
Social Workers
Social Workers are available to help you and your family deal with a wide range of issues. They are experts in communication and conflict resolution. They have a vast array of resources in the community that they can make available to you for non-medical issues.
Hospice Aides
Certified Nursing Assistants will visit you regularly to provide personal care and support for you and your family. They can help with many things, including personal care such as bathing, grooming, and dressing, and also offer companionship during their visits.
Specialized Therapists
Specialized therapists are available to all hospice patients when the hospice team has identified a need. These include Physical, Occupational, Speech, Pet, and Music therapists who can help you through your particular challenges.
Bereavement Coordinator
The end-of-life process is one of the most challenging things you will experience. We understand this, and our Bereavement Coordinators are there to guide you every step of the way. They will educate you and help you prepare for each phase involved in the process. They also continue providing care, support, and grief counseling to family members for up to 13 months.
Volunteers
Our army of trained volunteers is available to provide companionship and support to you and your family. They visit periodically, spending quality time with you, reading, reminiscing, providing simple caregiving services, and may occasionally help with some light household chores.
